Note that while the Wufoo hosted data collection service is not free, they clearly say that you can design forms etc. using their templates and themes and then download the generated HTML and CSS. Just be sure to properly indentify Wufoo as the source of the code (even if you tweak it) and give credit where it is due.

The replacement of the numbering with an infographic for the themes is a fantastic example of information communication. In a very small space they have shown the color pallet and a high-level style guide indentifying what the colors are used for. All that is easily understood at a glance. Bravo! I would be tempted to add a legend as a tooltip to explain what each letter represents. A full legend is available on the CSS Themes page itself.
